Please help us to find the solution to deal with Concrete edge failure when anchor bolt set on RC beam.

I am dealing with a case for anchor bolts setting on R.C. beam, however it seems impossible to solve the concrete edge failure, could you help me to find out the solution?Thanks!

Noted that the upper edge of plate to beam edge is limited to 250mm, so the plate and bolts cant move upwards anymore. And in this case the loads value had been multiplied by 2 under SOFA method to satify the F.O.S.3
